Ongoing Social Welfare & private certificates can be requested using the online form below. Please note only continuation certs that have been discussed with the doctor can be ordered here.  Phone (091) 842144 if it is a new medical condition. Patients will be reviewed at the request of the GP and you must present when requested for cert review.

Note: there is a €20 fee for private work certificates.
